Hair Transplant is one of the commonest cosmetic surgeries done by cosmetic surgeons / hair transplant surgeons in their daily practice. Standard hair restoration methods have emerged as a boon for patient of hair loss, specially for patients with hair fall in their heads or more commonly-- the bald people.
A meeting session of the patient with the Hair Transplant Surgeon, is conducted 1 to 2 weeks before Hair Transplant Surgery; to know the expectation of the patient & to determine the surgical plan. During this, Preoperative instructions are explained to the patients. They are instructed to discontinue medications and diet supplement, that can cause bleeding problems such as aspirin, anticoagulants, ibuprofen type anti inflamatories, vitamin E, garlic pills, fish oil capsules, herbal supplements; cigarette smoking should be stopped at least 2weeks before Hair transplantation surgery. The patients are instructed to wash their hair the day before the hair surgery and on the morning of the surgery by an Antiseptic shampoo.
The Hair-Surgery is done under local anaesthesia, as a Day Care procedure. The patient should be accompanied by an attendent; as after the surgery although the patient would be able to drive back home, but it is not advisable.
There are 2 different types of hair Restoration Technique, FUT (Follicular Unit Transplantation)and FUE (Follicular Unit Extraction)
In Follicular Unit Transplantation (FUT) also known as Strip Technique; the donor site is usually the back of the head, where the hairs which are resistant to loss with ageing, are present. Local Anaesthesia is infiltrated in the area chosen, for harvesting the donor hair graft. By surgical incision, a strip of variable area of scalp containing hairs is taken from back of the head. After securing haemostasis, the wound is closed by sutures. These thin strips are then easily micro dissected into grafts, by a technique called Slivering. These grafts are then transplanted in the anaesthetised, desired baldness area. Advantages of this procedure are --1) it is considered to be the gold standard of hair transplantation. A vast majority of hair implantation in the world is done by this method 2) It is quicker 3) it is cheaper 4) More larger area can be transplanted 5) Less loss of donor hair graft, as less chance of transection is present. The Disadvantage is it needs a suture in back of the head.
In Follicular Unit Extraction (FUE), the surgeon uses 1 mm punch, to excise individual follicular units, from the donor area. After the punch is removed, a tissue grasping forceps is used to extract the individual(one to four hairs) unit. The Advantages of this hair transplantation procedure is, it avoids the linear scar in the back of the head. But Disadvantage of FUE hair transplantation Technique is 1) There is more chance of damage to follicular bulb of the donor hair 2) The time duration of FUE Hair transplant surgery increases 3) It is also more costly 4) Smaller transplants than the standard strip method are possible.
